“Redo” tells a team what happened, but not what to improve. Give each reworked order one primary reason code, then add a short note for context.
Keep the list small and observable
Useful categories might include customer text changed, artwork approval missed, wrong blank selected, setup reference lost, or visible result rejected. Choose only a category supported by the order record. Do not assign a machine or material cause without evidence.
One primary code keeps weekly counts readable. A secondary note can preserve nuance without turning every event into a unique category.
Review process, not people
Look for repeated handoff failures and unclear checkpoints. If many orders lack a recorded approval, fix that step before drawing conclusions about output quality. Rework records should support learning rather than blame.
